Individual Registration Form, Waiver of Liability
1. Waiver Information
1.1. Conditions of Participation: The following terms apply to the EnduRow Challenge Event, organised by EnduRow Challenge involving virtual participation, rowing machines at various independently coordinated and supervised venues known as MacRow events which are operating outside the scope of responsibility of EnduRow Challenge and belonging to individual participants known as MicRow events which are operating outside the scope of responsibility of EnduRow Challenge. This is a binding legal document detailing the Conditions of Participation, Acknowledgement of Risk and Waiver of Liability. EnduRow Challenge, MacRow venue hosts and MicRow venue hosts assume no liability for participants and all participants agree to responsibility for their own safety and well-being. MacRow hosts and MicRow hosts have no official or legal affiliation with EnduRow Challenge and EnduRow Challenge assumes no liability in association with either.
1.2. Personal Conduct:
- Participants are expected to listen and take note of the Safety Briefing at the start of the event.
- Participants must obey instructions from EnduRow Challenge staff in order for the event to run safely and smoothly.
1.3. Safety:
- Providing a safe and enjoyable environment is key to a safe and smooth event.
- All coaches used to deliver sessions are appropriately qualified, are to be DBS checked and first aid trained.
- All participants are to be supervised and assisted as needed throughout the event.
1.4. Clothing: All participants are to wear appropriate clothing and footwear as advised by EnduRow Challenge.
1.5. Breach of the above requirements or inappropriate behaviour means participants may be excluded from the remainder of and/or further sessions.
1.6. Photography: Consent for use by EnduRow Challenge for publicity and marketing purposes of any photographs, images and film taken during the session, in which you may be visible, will be deemed to have automatically been given by completion of this form. Please indicate to your coach if you would like to withdraw this consent.
1.7. Health Declaration: EnduRow Challenge endeavours to be as inclusive as is reasonably practical and safe. Participants must disclose any know illness or medical conditions. Please provide details if you suffer from epilepsy, giddy spells, asthma, diabetes, heart condition or anything else you believe may affect you during your time with us. Participants must bring any prescribed medication with them to the sessions and declare its location to the coach.
1.8. Acknowledgement of Risk: I understand that there are foreseeable and unforeseeable risks associated with using rowing machines and partaking in this event. I understand that there is a risk of injury, disability and death despite all efforts made by EnduRow Challenge to minimise such risk.
1.9. Waiver of Liability: I acknowledge that I have read, fully understood and am satisfied with the details supplied about the activity and the associated risks. I agree without any limitation whatsoever that I am responsible when taking part in the session. I declare that I know of no medical reason why I should not participate and I am happy with arrangements outlined in the conditions of participation and fully acknowledge the risks noted above.
I agree that:
- EnduRow Challenge only provides insurance cover against proven negligence by staff and volunteers. I should consider making my own arrangements for personal accident cover for I am responsible.
- Except in respect of death or personal injury caused by the negligence of EnduRow Challenge employees, volunteers or agents, I hereby waive, release and indemnify EnduRow Challenge and their employees, agents and volunteers of any and all claims, however arising whether in contract or for personal injury, property damage, and any losses whatsoever that may arise from participating in the activity.
- EnduRow Challenge shall not be liable, and I hereby waive all claims, for any damage to, or loss of personal property.
- I am responsible for disclosing any known illness or medical conditions to ensure that the coaches are correctly briefed. EnduRow Challenge reserves the right to exclude anyone from a session or sessions due to concerns on health, medical conditions or behaviour, at their absolute discretion and without any recourse.
- EnduRow Challenge will not provide any adult supervision of the participant outside of a session. The participant agrees to attend the whole session, gaining prior consent of the coach before joining or leaving a session.
2. Code of Conduct
These rules protect the participant and other users from damaging equipment and putting themselves in danger whilst participating in EnduRow Challenge activity.
Rowing Activity
Unless instructed otherwise by EnduRow Challenge Staff:
I will not use the equipment without consent and supervision from an approved EnduRow Challenge Coach
2. I will wear suitable clothing for the activity and will make sure I come prepared with food and water
3. I will observe the code of practice at all times for the equipment that I am using which will be explained by the coach
4. I will treat the equipment with respect and make sure it is clean and stored correctly after use
5. I understand the possible dangers of participating in activities on rowing machines and I will act appropriately in order to keep myself and others safe
6. I will respect mentors, volunteers and EnduRow Challenge staff by listening and following instructions carefully
7. I will respect others by treating them fairly to allow them to participate in an open and friendly environment
8. I will ensure sessions can run as planned by turning up on time and prepared for the session
3. Declaration
By completing the registration form, I acknowledge that I have read, fully understood and are satisfied with the details supplied about the activity and the associated risks. I agree without any limitation whatsoever to take part in sessions with EnduRow Challenge.
Whenever I execute an electronic signature, it has the same validity and meaning as a handwritten signature. I will not at any time in the future, repudiate the meaning of my electronic signature or claim that my electronic signature is not legally binding.
- I have received details of the activity via email, phone call or letter and consent to participating
- I have read, understand and consent to the conditions of participation, acknowledgement of risk and waiver of liability
- I confirm that I understand the code of conduct and acknowledge that serious misconduct or repeated bad behaviour will result in their removal from the event
- I agree to give EnduRow Challenge permission to authorise medical treatment and any emergency dental, medical, or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present if required in the event of an emergency
- I agree that photographs and video may be made of me for use in promotional material for EnduRow Challenge only

All data provided on this form by participants will be held in confidence and in accordance with the Data Protection Act 1998 for seven years from the date of signature.
